I.Tóm tắt Nghiên cứu PSAT1 trong Ung thư Vú
Nghiên cứu này điều tra sâu rộng về phosphoserine aminotransferase 1 (PSAT1) trong quá trình tiến triển của ung thư vú. Luận án tập trung vào vai trò của PSAT1 trong di căn ung thư vú ba âm tính (TNBC) và kháng thuốc nội tiết ở ung thư vú dương tính với thụ thể estrogen (ER+BC). Ung thư vú là chẩn đoán khối u phổ biến nhất ở phụ nữ. Tỷ lệ sống sót sau 5 năm tổng thể đạt 90%. Tuy nhiên, tỷ lệ sống sót sau 5 năm đối với bệnh di căn chỉ là 22%. Di căn và kháng thuốc nội tiết ảnh hưởng đến hơn 50% bệnh nhân. Cả hai yếu tố này đều làm giảm đáng kể tiên lượng bệnh. PSAT1 cùng với con đường tổng hợp serine (SSP) có liên quan đến cả di căn và kháng thuốc nội tiết. Nghiên cứu này khám phá cơ chế tiềm ẩn của PSAT1. Mục tiêu là xác định khả năng của PSAT1 như một mục tiêu điều trị mới cho bệnh nhân ung thư vú. Các phát hiện cung cấp cái nhìn sâu sắc về sinh học khối u.
1.1. Mục tiêu chính của luận án
Luận án tập trung làm rõ sự tham gia của PSAT1 trong quá trình tiến triển ung thư vú. Mục tiêu cụ thể là nghiên cứu PSAT1 trong di căn TNBC. Một mục tiêu khác là khám phá vai trò của PSAT1 trong kháng thuốc nội tiết ở ER+BC. Giả thuyết đặt ra là PSAT1 đóng vai trò quan trọng trong di căn TNBC. PSAT1 cũng được cho là góp phần vào sự kháng thuốc nội tiết ở ER+BC. Các nghiên cứu trước và dữ liệu sơ bộ từ phòng thí nghiệm đã gợi ý điều này. Các nhà khoa học đã thiết kế các thí nghiệm cụ thể. Họ đã thay đổi biểu hiện PSAT1 trong các dòng tế bào ung thư vú khác nhau. Các phát hiện mới cung cấp cái nhìn sâu sắc về sinh học ung thư vú. 1.2. Bối cảnh và vấn đề ung thư vú
Ung thư vú vẫn là một thách thức lớn trong y học. Đây là loại ung thư phổ biến nhất ở phụ nữ. Tỷ lệ sống sót sau 5 năm nói chung là rất cao. Tuy nhiên, bệnh di căn lại có tỷ lệ sống sót rất thấp. Hơn một nửa số bệnh nhân phải đối mặt với di căn hoặc kháng thuốc. Nhu cầu tìm kiếm các mục tiêu điều trị mới là cấp thiết. PSAT1 nổi lên như một yếu tố tiềm năng. Nó có thể tác động đến cả di căn và kháng thuốc. Việc hiểu rõ hơn về PSAT1 có thể mở ra hướng điều trị mới.
II.Vai trò PSAT1 trong Di căn Ung thư Vú Ba Âm tính TNBC
Ung thư vú ba âm tính (TNBC) là một dạng ung thư vú đặc biệt hung hãn. TNBC không có các thụ thể hormone estrogen, progesterone và HER2. Điều này làm cho việc điều trị trở nên khó khăn hơn. PSAT1 đã được xác định là một yếu tố quan trọng trong di căn TNBC. Nghiên cứu đánh giá tác động của việc thay đổi biểu hiện PSAT1. Các dòng tế bào TNBC “không phụ thuộc vào tổng hợp serine” được sử dụng. Việc ức chế PSAT1 mang lại kết quả đáng chú ý. Nó làm giảm đáng kể khả năng di chuyển và xâm lấn của tế bào. Kết quả này không được lặp lại khi ức chế PHGDH. PHGDH là enzym đầu tiên trong con đường tổng hợp serine. Điều này cho thấy PSAT1 có thể có vai trò độc lập. Nó không chỉ đơn thuần là một phần của con đường sinh tổng hợp serine truyền thống. Các phát hiện này mở ra hướng điều trị mới cho TNBC. Các nghiên cứu tiếp theo sẽ tập trung vào cơ chế này.
2.1. Ảnh hưởng của PSAT1 lên khả năng di chuyển và xâm lấn tế bào

III.PSAT1 và Kháng thuốc Nội tiết ở Ung thư Vú ER
Ung thư vú dương tính với thụ thể estrogen (ER+BC) thường được điều trị bằng liệu pháp nội tiết. Tuy nhiên, kháng thuốc nội tiết là một thách thức lớn. Nó dẫn đến tái phát và bệnh tiến triển. Nghiên cứu này cũng xem xét vai trò của PSAT1 trong vấn đề này. PSAT1 và PHGDH đã được chứng minh có mối tương quan. Mức độ biểu hiện cao của cả hai enzyme này liên quan đến tiên lượng xấu. 3.1. Mối tương quan của PSAT1 và PHGDH với tiên lượng sống còn

Việc giám sát mức độ PSAT1 và PHGDH có thể hữu ích. Nó có thể giúp định hướng chiến lược điều trị cá nhân hóa. Điều này hỗ trợ việc lựa chọn phác đồ hiệu quả hơn.
3.2. Thay đổi độ nhạy cảm với Tamoxifen qua PSAT1 PHGDH
Nghiên cứu đã thực hiện các thí nghiệm trên dòng tế bào. Các dòng tế bào ER+BC nhạy cảm và kháng thuốc được dùng. Thay đổi mức độ biểu hiện của PSAT1 hoặc PHGDH. Việc này đã ảnh hưởng trực tiếp đến độ nhạy cảm với 4-hydroxytamoxifen. 4-hydroxytamoxifen là chất chuyển hóa hoạt động của tamoxifen. Khi PSAT1 hoặc PHGDH bị ức chế, độ nhạy cảm tăng lên. Điều này có nghĩa là tế bào ung thư trở nên dễ bị tiêu diệt hơn. Ngược lại, khi biểu hiện tăng, tế bào có thể trở nên kháng thuốc hơn. Những kết quả này cung cấp bằng chứng cơ bản. Chúng cho thấy việc nhắm mục tiêu vào các enzyme này. Nó có thể khôi phục lại độ nhạy cảm với liệu pháp nội tiết. Điều này có thể giúp giải quyết vấn đề kháng thuốc. IV.PSAT1 Mục tiêu Điều trị Ung thư Vú Tiềm năng
Các phát hiện từ nghiên cứu này đã củng cố vai trò quan trọng của PSAT1. PSAT1 tham gia vào cả di căn TNBC và kháng thuốc nội tiết ER+BC. Điều này biến PSAT1 thành một mục tiêu điều trị hấp dẫn. PSAT1 là một enzyme trung tâm trong chuyển hóa serine. Chuyển hóa serine đóng vai trò thiết yếu trong sự tăng trưởng của tế bào ung thư. Đặc biệt là trong các tế bào tăng sinh nhanh chóng. Việc nhắm mục tiêu vào PSAT1 có thể ức chế sự phát triển của khối u. Nó cũng có thể ngăn chặn sự lây lan của tế bào ung thư. Các phương pháp điều trị mới có thể được phát triển. Các phương pháp này sẽ tập trung vào việc điều hòa hoạt động của PSAT1. Điều này mở ra một hướng mới cho việc quản lý ung thư vú. Đặc biệt là đối với các dạng ung thư vú khó điều trị. Nghiên cứu này có ý nghĩa lâm sàng quan trọng.
4.1. PSAT1 và chuyển hóa serine trong tế bào ung thư
PSAT1 là phosphoserine aminotransferase 1. Nó là một enzyme chủ chốt trong con đường sinh tổng hợp serine. Con đường này cung cấp serine và glycine cho tế bào. Serine và glycine là các axit amin cần thiết. Chúng cần cho tổng hợp protein, nucleotide và lipid. Tế bào ung thư có nhu cầu trao đổi chất cao. Chúng sử dụng con đường sinh tổng hợp serine để duy trì sự tăng sinh. PSAT1 thúc đẩy chuyển hóa tế bào ung thư. Nó hỗ trợ sự tăng trưởng và sống sót của tế bào. Việc ức chế PSAT1 có thể làm gián đoạn nguồn cung cấp các chất cần thiết này. Điều này có thể làm chậm hoặc dừng sự phát triển của khối u. Nó đặc biệt hiệu quả trong các loại ung thư vú có biểu hiện cao PSAT1. PSAT1 là một phần quan trọng trong chuyển hóa tế bào ung thư.
4.2. Tiềm năng của PSAT1 như một dấu ấn sinh học
PSAT1 không chỉ là một mục tiêu điều trị tiềm năng. Nó còn có thể đóng vai trò là dấu ấn sinh học tiên lượng ung thư vú. Mức độ biểu hiện của PSAT1 có thể cung cấp thông tin quý giá. Nó có thể dự đoán sự tiến triển của bệnh và khả năng đáp ứng điều trị. Đặc biệt, PSAT1 có thể giúp xác định bệnh nhân có nguy cơ cao di căn. Nó cũng có thể nhận diện bệnh nhân có khả năng kháng thuốc hóa trị hoặc nội tiết. Việc sử dụng PSAT1 làm dấu ấn sinh học có thể cá nhân hóa điều trị. Nó giúp bác sĩ lựa chọn liệu pháp phù hợp nhất. Điều này cải thiện hiệu quả điều trị và kết quả cho bệnh nhân. PSAT1 có thể trở thành công cụ chẩn đoán quan trọng trong tương lai.

iv ABSTRACT INVESTIGATION OF PHOSPHOSERINE AMINOTRANSFERASE 1(PSAT1) IN BREAST CANCER PROGRESSION Stephanie Metcalf August 23, 2019 This dissertation describes my research into the involvement of phosphoserine aminotransferase 1 (PSAT1) in breast cancer progression; specifically, in triple negative breast cancer (TNBC) metastasis and endocrine resistance in estrogen receptor positive breast cancer (ER+BC). Breast cancer is the most common tumor diagnosis among women. While the overall 5-year survival for breast cancer is reaching 90%, the 5-year survival for metastatic disease is only 22%. Metastasis and endocrine resistance combined can affect over 50% of patients.
One of the proteins and pathways implicated in both metastasis and endocrine resistance in breast cancer is phosphoserine aminotransferase 1 (PSAT1) and the serine synthetic pathway (SSP). From prior reports and preliminary studies within the lab, I hypothesized that PSAT1 may play a role in metastasis within TNBC and contribute to endocrine v within ER+BC. The role of PSAT1 in TNBC metastasis was evaluated via examination of the effects of altered PSAT1 expression on metastatic potential in TNBC cell lines that were “serine synthesis-independent”. Functional relevance of PSAT1 on sensitivity to endocrine therapy was tested in matched endocrine sensitive and endocrine resistant cell lines upon alteration of PSAT1 expression.
Through this work, I found that suppression of PSAT1 significantly inhibited the in vitro motility and invasiveness of “serine synthesis-independent” TNBC which was not recapitulated upon suppression of PHGDH, which is the first enzyme within the SSP. I also found that suppression of PSAT1 reduced the number of micro-metastases within the lungs in an experimental metastasis model. In addition, I found that both PSAT1 and PHGDH correlated to poorer progression free survival in multiple patient cohorts, manipulation of PSAT1 or PHGDH in both sensitive and resistant ER+BC cell lines altered sensitivity to 4-hydroxytamoxifen treatment. This body of work has demonstrated that PSAT1 selectively promotes metastasis in “serine synthesis-independent” TNBC via a function unrelated to de novo serine synthesis.
It also has shown that both PSAT1 and PHGDH contribute to tamoxifen resistance in ER+BC and thereby implicating a role for the SSP in this context. Model of SSP Contribution to Endocrine Resistance………………………… 96 xi CHAPTER 1 INTRODUCTION Breast cancer is the most common malignancy in U. women, constituting 30% of all new cancer diagnoses. As the second leading cause of cancer-related deaths in women [1], it comprised 6.8% of all cancer related mortalities in 2016 [2]. The lifetime risk for a woman developing breast cancer is currently 1 in 8 or about 12% [2].
It is estimated that 268,600 women will be diagnosed with invasive breast cancer while another 62,930 will be diagnosed with non-invasive breast cancer this year [1]. At the beginning of 2019 there were in excess of 3.1 million women in the U. with a history of breast cancer [1]. These women though all diagnosed with breast cancer, do not all have the same disease.
Breast cancer has been classified into 21 different histological subtypes that can be categorized into four main categories based on their hormone receptor (HR) and human epidermal growth factor receptor 2 (HER2) status [3]. The four main subtypes are luminal A (HR+/HER2-), luminal B (HR+/HER2+), triple negative (HR-/HER2-), and HER2 enriched (HR-/HER2+) [3]. These all have different clinical presentations, oncogenic drivers, treatment options, affected patient populations, and confer different patient prognoses [3]. This dissertation will focus on two of these subtypes; triple negative breast cancer (TNBC) and luminal A, which will be referred to as estrogen receptor positive breast cancer (ER+BC).
1 TNBC is characterized by lack of estrogen receptor-α (ERα), progesterone receptor (PR), and HER2 expression [4]. This molecular subtype accounts for 10% to 20% of all breast cancer cases [3,5]. It is often considered to be the most aggressive with the majority of these tumors being high grade at time of diagnosis and basal-like in their appearance [6]. This aggressiveness is especially alarming considering the patient populations most commonly diagnosed with this form of cancer.
TNBC is most common among women under 50 years of age, among women that are African American or Hispanic, and among women that harbor breast cancer gene 1 (BRAC1) mutations [3,7,8] (Figure 1). The aggressiveness of TNBC could be due to its heterogeneous nature and the limited treatment options, given that targeted therapies for breast cancer primarily focus on ERα and HER2. TNBCs are currently treated with a combination of surgery, chemotherapy and radiation [3]. Given the propensity of TNBC to be of a higher grade and more aggressive, there is an increased likelihood of metastasis within this subtype [5].
ER+BC is the most common type of breast cancer observed in the clinic, accounting for approximately 60% to 75% of all breast cancer diagnoses [9]. These tumors express ERα and their growth and proliferation is driven by estrogen [10]. ER+ tumors are typically slow-growing and found in older patients, as most patients with ER+BC are post-menopausal [7]. This subtype also has the best prognosis, especially at early stages, due to targeted therapies that have been developed [3] (Figure 1).
These therapies target different aspects of estrogen processing and can be classified into three major categories: aromatase inhibitors, selective estrogen receptor modulator (SERM), and selective estrogen receptor 2 Figure 1: General subtypes of Breast Cancer. The different categories of endocrine therapy all target estrogen but do so via several different mechanisms (Figure 2). Aromatase Inhibitors, such as letrozole, prevent the conversion of androgens to estrogens [11]. Tamoxifen is a SERM and its primary mechanism of action is through competitive binding against estrogens for the estrogen receptor resulting in the inhibition of the estrogen driven pro-proliferative transcription program [12,13]. Fulvestrant binds to the estrogen receptor and attenuates estrogen signaling through blocking estrogen binding and increased ER protein degradation [12].
Tamoxifen was the first of these targeted therapies and became the standard first-line therapy in the 1990s [9]. Since its introduction, the mortality rates associated with breast cancer have decreased significantly [3]. Much of this trend has been attributed to Tamoxifen, and its later designated replacements, that target estrogen signaling in ER+ tumors as well as improvements in detection methods [3]. With these advances in therapies and detection techniques, the current overall survival rate for female breast cancer has improved and is approaching 90%; however, metastatic disease, where the overall 5-year survival rate is 22% [2] is still an overwhelming problem.
Metastases or advanced recurrent disease will develop in approximately 30% of patients despite treatment with standard and advanced therapies [14] and is responsible for 90% of breast cancer-related deaths [15].
